Daniel Horn, M.D., is a primary care doctor, writer, and population health implementation expert. He is currently the director of population health and quality for the Division of General Internal Medicine at Massachusetts General Hospital.

As an active primary care doctor, I provide preventive, chronic disease and urgent care for patients at Massachusetts General Hospital. I also regularly serve as a teaching attending on the general medicine inpatient floors.

I write in both the lay and academic press about healthcare innovation, care redesign and pressing health policy issues in the United States.
Implementer
I lead efforts to improve the quality of the chronic disease and preventive care at Massachusetts General primary care practices. I also oversee our telehealth, behavioral health integration, and high-risk management programs for primary care. Research here.
